An Initial Evaluation is required prior to creating a treatment plan.
Fern Physical Therapy is an out-of-network provider and does not have a preferred provider/contracted status with any insurance company. As a result, patients will receive optimal patient care, and the therapist is not forced to limit the quality of treatment or the time of treatment due to insurance company restrictions. Fern Physical Therapy rates are reflective of the provider’s experience and education, competitive in the market, and are not elevated to pay for administrative personnel or billing services. An invoice for services received will be provided at the patient’s request, and the patient can submit the invoice to insurance for reimbursement.
No Surprises Act & Good Faith Estimate
The No Surprises Act is a law that requires health care providers to provide self-pay patients (patients who are not submitting to insurance) with a good faith estimate. You have the right to request the total expected cost & amount of sessions needed prior to scheduling services. If requested, Fern Physical Therapy will provide a Good Faith Estimate.
Reschedule & Cancellation Policy
You are signing up for a highly personalized service at a small business. You will be showered with attention & care during your sessions, & will typically require fewer sessions than at a mainstream PT clinic. Because each session is special, there is a 50% charge fee ($80 for evaluations & $60 for follow up sessions) for any appointment canceled or rescheduled within 24 hours of the appointment time. We would much rather treat you than charge this fee, so please schedule your appointments thoughtfully & plan to attend all sessions.
