No Surprises Act & Good Faith Estimate

The No Surprises Act is a law that requires health care providers to provide self-pay patients (patients who are not submitting to insurance) with a good faith estimate. You have the right to request the total expected cost & amount of sessions needed prior to scheduling services. If requested, Fern Physical Therapy will provide a Good Faith Estimate.

Reschedule & Cancellation Policy

You are signing up for a highly personalized service at a small business. You will be showered with attention & care during your sessions, & will typically require fewer sessions than at a mainstream PT clinic. Because each session is special, there is a full charge fee ($150 for evaluations & $110 for follow up sessions) for any appointment canceled or rescheduled within 48 hours of the appointment time. We would much rather treat you than charge this fee, so please schedule your appointments thoughtfully & plan to attend all sessions.
